New Patient Registration
HOW TO REGISTER WITH THE PRACTICE
We have an open list and would be delighted to welcome you as a patient if you live within our practice area. We register new patients between 09.00 – 17.00. You will be asked to make an appointment to see one of our nursing team(optional)
Our staff are happy to help you with the process - to register you must bring the following documentation:
Place of birth – resident of UK:
- Birth certificate accompanied by photographic ID (passport, driving licence, ID card)
- Proof of your current address (mortgage/rental agreement, utility bill within last 3 months, council tax documents, current employment documents)
Place of birth – out with the UK or UK passport holder resident out with the UK for a period of time:
- Passport
- Visas
- Proof of your current address and any relevant documents required to show that you are entitled to NHS treatment
- Students- we require to see a letter from your university/college which confirms you have a place on a full-time course (for example your student visa plus letter)
Please note you cannot register without proof of entitlement to NHS treatment.
In addition, all patients will be asked to complete a new patient registration form and questionnaire, allowing us to provide medical care in the interim period while your records are transferred from your old practice. You will be offered an appointment for a brief health check at the earliest convenience.
Preference of GP
Patients are registered with the practice and not an individual GP. For administrative reasons your medical card will be issued in the name of one of the doctors; however, you can at any time express a preference for a particular doctor.
